Job Description

Position Summary: Using a “foster care model” the Host Family/Live-In Manager has overall responsibility for managing and maintaining the home and providing daily care and supervision for home’s residents. The Manager is responsible for the resident/s 24/7. S/he provides management, instruction, guidance, support, and assistance with problem-solving and activities of daily living. The specific responsibilities and duties are determined by the individual needs and preferences of the resident(s) in the home.

 

General Responsibilities:

 

Provide instruction and assistance with scheduling of daily activities and routines such as:

  • The primary caregiver must reside full-time at the address as the person(s) receiving services and must not leave the individual(s) alone.
  • Meal planning and preparation
  • Maintaining appropriate personal appearance
  • Maintaining a clean and orderly home environment
  • Following schedules as required
  • Medication
  • Ensure transportation is provided for resident(s) day programs, activities, employment and appointments, as needed.
  • Money management
  • Shopping
  • Community activities
  • Utilization of community resources
  • Participate in planning for the resident(s) receiving services.
  • Participate with and assist resident(s) to develop and expand participation in typical community activities and to build relationships with family, friends and neighbors.
  • Participate in staff meetings, required training and supervision meetings
  • Complete required documentation/ file maintenance
  • Participate in required reporting to funding and regulatory bodies.
  • The Manager must never leave the resident(s) receiving supports alone.
  • Participate in the development of and implement services as specified in the individual service plan.
  • Report injuries, illness and/or accidents involving residents to Assistant Director, per agency policy and procedure and in accordance with DHS Office of Inspector General.
  • Cooperate with the background inquiry process required for licensing, re-licensing and/or certification.
  • Ensure the resident(s) receives required medical and dental care in a timely manner.
  • Actively participate in service planning as a member of the interdisciplinary team by attending all service plan meetings, special meetings and completing monthly progress review reports.
  • It is expected that the Host family will have the ability to work effectively with people with disabilities.  It is important that the Manager is highly sensitive to the needs and preferences of the individuals they are providing services.  The following are other criteria that need to be met:
    • Problem-solves and works cooperatively with persons with disabilities, families/guardians and other staff.
    • Ability to establish a relationship with the individual supported that is based on advocacy and support, rather than supervision or control
    • Ability to respond well to crisis situations

 

Relief Time:

 

Twenty hours of relief will be provided each week, with scheduling to be mutually agreed upon.

In general, the Manager will be free while resident(s) attends day programming (approximately from 9:00 AM to 3:00 PM Monday through Friday, depending on program hours). On days that resident(s) is not able to attend programming the Manager must be able to stay home with resident(s).

Requirements

Qualifications:  High school diploma with a minimum of one year of experience working directly with persons with Intellectual Disabilities. Experience with Autism is preferred. A valid driver’s license is required and the ability to meet agency insurance requirements.  Supervisory experience and experience in a residential setting preferred.
