Job Description

Supervision-

  • Interview and hire staff who work directly under this position
  • Provide orientation, training, supervision, annual evaluation, and discipline of staff
  • Ensure completion of On-the-Job Training (OJT) staff and Direct Support Professional (DSP) staff training within the 120-day time frame as required by DHS
  • Ensure that staff completes all agency and DHS required and recommended training within 120 days for new hires and annually for all other staff
  • Provide appropriate model of client interaction with the staff of the program and other programs
  • Be available as a resource person to staff and contribute to their ability to provide services through training and supervision
  • Oversee, model, and supervise direct service staff
  • Assume all direct service responsibilities when coverage cannot be found, including driving agency vehicle and transporting clients
  • Complete monthly reports on home environment, making recommendations through budgetary perspectives
  • Ensure incident reports are completed and reviewed within 24 hours
  • Chair bi-monthly home meetings with all staff and complete meeting minutes; ensure minutes are signed by staff members and scanned into the H:// Drive
  • Complete weekly Basic Assurance of Medications checks and submit to nurse
  • Support the manager On-Call team with clinical and emergency issues on a 24-hour basis 
  • Carry On-Call cell phone as assigned
  • Know and keep staff informed of all agency policies, procedures, and changes as they occur 
  • Keep staff informed of agency information, updates, and any other changes
  • Monitor staff time daily on Easy Clocking
  • Submit staff payroll print-out to Payroll Department on Payroll Mondays
  • Monitor client medication daily, report medication errors to nurse immediately
  • Work one weekend day per month
  • Visit house during off-shift times twice month

Client Services-

  • Coordinate client intake, admission, transfers and discharge in conjunction with the ISC
  • Ensure that monthly client meetings are held where client preferences are discussed with regards to outings, activities, meals, etc.
  • Ensure that a weekly activity schedule is maintained for clients that incorporates client wishes, wants, and input
  • Intervene in problem or crisis situations as required
  • Confirm daily and weekly data is being tracked for each client in regards to ISP goals and behavior data
  • Work in collaboration with Board Certified Behavior Analyst (BCBA) or Behavior Analysist to create and implement behavioral plans twice a year
  • Collaborate with the BCBA and other team members to improve services for the clients and promote continuity across environments
  • Coordinate and monitor health care services for each client in conjunction with the Medical Services team
  • Ensure Nursing Service Packets with PSR are completed annually, within Department of Human Services (DHS) required guidelines and time frames and submitted to DHS
  • Coordinate, monitor and evaluate the delivery of services for each individual client
  • Ensure Individual Service Plan and/or review is completed at least 7 days before the due date and posted on the G://drive for review

Property-

  • Coordinate maintenance of the building and ensure follow-up on maintenance issues
  • Conduct and submit quarterly physical plant inspections to Residential Director
  • Report issues to Maintenance that need attention within 24 hours of discovery
  • Coordinate with Maintenance the regular upkeep of interior and exterior areas of the building
  • Oversee van maintenance including coordinating repairs and preventive maintenance in collaboration with Maintenance
  • Keep vehicles clean and odor free, exterior and interior

 

Fiscal-

 

  • Complete and submit monthly client attendance and money reconciliations by the 1st of the month and submit to Accounting
  • Maintain current and accurate financial records and coordinate the appropriate expenditure of program and client monies as well as LINK cards
  • Coordinate the purchasing of needed program and facility supplies, equipment and furnishings
  • Adhere to Rule 115 and 116
  • Monitor client
    • bank accounts,
    • LINK eligibility,
    • Medicaid eligibility, and
    • SSA eligibility and monies
  • Complete and submit copies of Medicaid and SSA documentation to Accounting Department
  • Submit Inventory for Client and Agency Planning (ICAP) scores to DHS rates department annually at the clients’ ISP date to get a rate adjustment

Other-

  • Maintain NCI, CPR and First Aid Certification
  • Participate in necessary trainings, meetings, and committees as needed or directed

Complete additional duties as assigned by supervisor

Requirements

Essential Skills and Experience:

  • BA/BS degree in social service related field
  • One year of experience working with persons with developmental disabilities
  • Valid Illinois driver’s license required
  • Ability to meet agency insurance requirements
  • Supervisory experience and experience in a residential setting preferred

Reporting to this Position:  All program staff

Normal work hours are 40 hours per week 10:00AM through 6:00PM, Monday through Friday. However, must maintain a flexible working profile while performing the job duties.
